Why QC might not pass (in real life)

QC is expired or prepared incorrectly
When QC is expired or prepared incorrectly we must either open a new vial of QC or use a new lot. If it was made incorrectly we must re-make the QC reagent according to proper guidelines and SOPs.

Equipment malfunction
Your equipment and analyzers might have had a tough time or are not working properly which means that you have to do some investigating and make sure that nothing is wrong with them.

Reagents used on analyzer are expired.
When reagents that are used on the analyzer to run QC and patient samples (such as slides that samples are distributed on in the Vitros) are expired they don't work correctly which impacts all samples ran with said reagent .

Improper QC reagent used
It's very easy to mix up the vials of QC that are used on an analyzer. You have to make sure that the solution you are using is the right thing you need by looking at the label and verifying it's correct.

All labs run things differently so make sure you know how your lab might troubleshoot or check with someone else.

Make sure your analyzer/equipment is working properly
You need to make sure your equipment isn't broken or malfunctioning and that it isn't causing any issues

Confirm the right QC vial was used
Make sure you used the right sample. Sometimes we have multiple different QC's and it's easy to mix them up!

Make sure that the QC hasn't expired. Expired QC will lead to wonky results.
